Seniors Well Aware Program (SWAP)

Provides outreach assessment, referral, treatment and follow-up to people over the age of 55 who may be experiencing problems due to the misuse/abuse of alcohol, prescription and and/or over the counter medications.  Also provides community awareness and education sessions to senior groups and health care professionals.  Open referral system.
